- If you require an emergency appointment please ensure calling
the surgery as early in the day as possible. Emergency appointments
will normally be made for the same day depending on your
availability. Emergency appointments may not be with your
regular dentist and may be with an emergency after hour’s
service.
- In the event of emergency appointments being made with
Pm-dent Ltd, the private emergency service, the relevant
fee would be paid by the patient and refunded by Dental Health
Club. The refund would only apply if temporary/pain relief
treatment were carried out. If permanent treatment is provided,
eg a filling or extraction, no refund would apply.
- At least 24 hours notice of cancellation of an appointment
is required. A charge of £10 per 15 minutes will be
levied for failed appointments or late cancellations.
- At the time of booking a new patient consultation a deposit
of £20 is charged. This fee is non-refundable if the
consultation is cancelled less than 24 hours of the appointment
time or if the appointment is not attended.
- Monthly subscription payments will normally be made by
direct debit on the first of each month. Alternatively, payments
can be made in advance for the following 12 months by cash,
cheque or credit/debit card.
- If payment for any treatment is not made for any reason,
entitlement to future treatment may then cease unless otherwise
arranged with the dentist. In all other circumstances entitlement
will recommence on payment of arrears.
- Full membership will cease if one or more monthly subscription
payments have been missed. To be entitled to the benefits
of the membership all subscription arrears must be paid.
- Cancellation of membership should be made in writing giving
one clear months notice.
- Treatment costs may, in some circumstances, be made in
stages as agreed with The Dental Health Club. MasterCard
and VISA are accepted.
- Where a patient is violent or abusive to any member of
staff, membership will be terminated immediately.
- Membership is not transferable to any other person.
- Children under 16 years of age can only be treated if accompanied
by a parent or guardian. No laser treatments can be carried
out on patients under the age of 18.
- The surgeries and reception areas are no smoking environments.
- No animals (except guide dogs) are permitted in the surgery
complex.
